The Faculty Association of SLSU, SLSU Employees Association, and SLSU Faculty and Employees Club are inviting reputable and duly licensed Health Maintenance Organization (HMO) providers to submit proposals for the provision of HMO-type benefits for qualified personnel of Southern Luzon State University (SLSU).
This initiative is in line with Section 7 of Executive Order No. 64, s. 2024, which authorizes, beginning FY 2025, the grant of a Medical Allowance in an amount not exceeding Seven Thousand Pesos (₱7,000.00) per annum to each qualified government civilian personnel.
Interested HMO providers must submit the following documents:
- Valid Business Permit
- PhilGEPS Registration (if available)
- SEC Registration
- Tax Clearance
- License or Accreditation from the Insurance Commission
HMO providers must also include in their submission:
- Detailed description of services and/or healthcare packages offered
- Coverage inclusions, exclusions, and benefit limits
- List of accredited hospitals, clinics, and medical practitioners
- Other relevant value-added services
