Important reminders, application requirements, program offerings, and examination schedules for Academic Year 2027-2028.
For all college applicantsThis guide applies to first-year and transferee applicants across all SLSU campuses. Read each section carefully before submitting an application.
Important Reminders
- 1.
All applicants, including transferees applying to any SLSU campus, must complete the online application form during the prescribed application period.
- 2.
Provide accurate personal information, academic history, family background, and other admission-related details. False information, intentional misrepresentation, or misdeclaration is grounds for disqualification.
- 3.
Upload all required documents:

3.b.Official academic record
3.c.Certificate of income of both parents or guardians
3.d.Certificate of residency
3.e.Additional requirements, when applicable
- 4.
Once submitted, the selected program is final and cannot be changed. Multiple submissions made to change the program choice will be disregarded; only the first application will be acknowledged.
- 5.
Rely only on announcements published through the official SLSU website, @slsuMain, and @SLSUAdmission. Disregard information from unauthorized individuals or unofficial pages.
- 6.
Updates regarding applications and examinations are available through the Application Status Portal section below.
- 7.
Bring and submit photocopies of all required documents on the scheduled examination date.
Application Requirements
Prepare clear and readable digital copies. Open each item below for the complete specifications. “View sample” links are provided only where an official sample is available.
1.Valid and active email address
Use an email account that you check regularly. Example: juandelacruz@gmail.com.
2.2×2 photograph with name tag
- Taken within the last six months
- Clear, forward-facing, and showing the full face
- Only the face and shoulders should be included
- Natural skin tone; no eyeglasses or colored contact lenses
- Ears visible when possible; avoid thick or dramatic makeup
- Colored 2×2 photograph with a white background
- Name tag format: SURNAME, FIRST NAME, MIDDLE INITIAL
- May use a digital or handheld name tag
- Must not be a scanned photograph from an existing valid ID
- JPG format with a maximum file size of 2 MB
3.Clear scanned copy of academic records
Grade 12 students expected to graduate: Grade 11 Report Card signed by the principal and class adviser. View two samples ↗
Senior High School graduates: Grade 12 Report Card signed by the principal and class adviser, with a GWA of 83 or better. View two samples ↗
Transferees and returnees: Transcript of Records or Copy of Grades. Applicants must not exceed two semesters and must have a GWA of 2.25, with no failing or incomplete grades. View sample ↗
ALS graduates: Certificate of Rating indicating “eligible for college admission.” View sample ↗
4.Certificate of income of both parents or guardians
Submit the document applicable to the family's circumstances:
4Ps members: MSWD/DSWD certification indicating the household number, applicant as beneficiary, and amount and source of income. View sample ↗
Indigenous Peoples members: Certification from the National Commission on Indigenous Peoples (NCIP). View sample ↗
E-CLIP or PAMANA beneficiaries: Certification from DILG or the Office of the Presidential Adviser on the Peace Process (OPAPP).
Parents employed in government or other agencies: BIR Form 2316 (year 2025) sample ↗; or Human Resource Office certification sample ↗ indicating salary and other compensation.
Parents working abroad: Overseas employment contract. View sample ↗
Parents who are business owners: BIR Form 1701A (year 2025). View sample ↗
Deceased parents: Death Certificate. View sample ↗
Solo parents: Solo Parent ID and Solo Parent Certificate from MSWD/DSWD, with the amount and source of income. View sample ↗
Other circumstances: For unemployed parents, housewives/househusbands, persons with no fixed income, guardianship, self-supporting students, skilled workers, retirees, or senior citizens, submit a notarized sworn statement indicating the names of parents or guardians and the amount and source of income. 5.Certificate of Residency
Submit a Certificate of Residency issued by the barangay, or a certification from the LGU identifying a Geographically Isolated and Disadvantaged Area (GIDA), when applicable. View sample ↗
6.Cultural, arts, or athletic participation documents, if applicable
Applicants who served as delegates or participants must submit at least one of the following:
School letter or certification signed by the principal and coach, identifying the activity and highest level reached. View sample ↗
Certification from a municipality or public/private institution identifying the activity and highest level reached. View sample ↗
Written endorsement from the Head of Sports and Cultural Program of the applicant's academic institution.
