Applications for Academic Year 2027-2028 will be accepted from September 9 to October 6, 2026. Applicants are advised to prepare all required documents before submission.

Application Requirements
- Accomplished Junior High School Online Application Form. The form will be available beginning September 9, 2026.
- Payment of the ₱150 examination fee.
- Three (3) copies of a recent 2×2 photograph with a name tag and white background.

One long white folder containing photocopies of the following:
- Form 137 or SF10 covering Grades 1-5, with the computed General Weighted Average (GWA), duly signed by the school principal
- Official Receipt (OR) for the examination fee
Reminder: Bring complete and legible documents. The Examination Permit will be released only after the submitted requirements have been verified.
Application Process
- Complete the Online Application Form, available beginning September 9, 2026.
- Wait for the payment schedule, which will be sent to the applicant's email address.
- Secure a payment slip from the Guidance Counseling and Testing Center, then proceed to the SLSU Accounting and Cashier's Office to pay the examination fee.
- Submit all application requirements to the SLSU Student Admission Office. After verification, the office will issue the Examination Permit.
- Take the examination on the assigned schedule. The time, venue, and other instructions will appear on the Examination Permit.
- Attend the interview with the Principal of the SLSU Laboratory School on the same day as the examination.
- Check the admission results through the official SLSU Facebook Page or the SLSU website.
